I understand that it is not the intent of the Care Coordination Program to receive PHI via text, and to the extent that I provide any such PHI to the Care Coordination Program via text, I agree to waive my right to health care information privacy and security and understand that a representative of the Care Coordination Program may redirect any such communication to a secure method.

The members of the Care Coordination Program may use the information collected through its services (including aggregated and de-identified participant data) for research and development purposes and to improve program performance. However, such usage will not include any personally identifiable information in such research or development.
Additionally, the Care Coordination Program websites are hosted in the United States of America and are subject to U.S. state and federal law.

Children
The Children’s Online Privacy Protection Act (“COPPA”) protects the online privacy of children under 13 years of age. If we learn that personal information has been collected from a user under 13 years of age, then we will take the appropriate steps to cause this information to be deleted. If you are the parent or legal guardian of a child under 13 who has joined this program or has otherwise transferred personal information, please contact GoMo Health using our contact information below to have that child’s account terminated and information deleted.
Text and E-mail
When a participant sends a text message with program-specific data, their mobile phone number and/or e-mail address, the full content of the message, attached files, or other information provided may be recorded. The Care Coordination Program may use this information to determine and send appropriate information or educational content to participants. Participants may indicate their preference to stop receiving communications or opt-out at any time. (See your choices regarding your personal information.)
You will receive messages periodically throughout the program period. Message & data rates may apply.
How to Opt-Out:
To opt-out from our SMS service, you can text STOP from your mobile phone to any text that you receive and we will unsubscribe you from the SMS text messaging program immediately. You will not receive any additional messages unless you re-register.

Information Collected Via Technology
As you use these pages, certain message delivery information may be passively collected and stored on the members of the Care Coordination Program’s or our service providers’ server logs, including your internet protocol address, browser type, and operating system. We may, when required, provide Personal Health Information to third-party service providers or affiliates who work on our behalf, to provide some of the services and features necessary for the Care Coordination Program. Examples of such services include sending texts or e-mails, analyzing data, and providing customer service.
Privacy Policy Changes
The members of the Care Coordination Program may, in their sole discretion, change this Privacy Policy from time to time. Any and all changes to the Privacy Policy will be reflected on this page and the date new versions are posted will be stated at the top of this Privacy Policy. Unless stated otherwise, our current Privacy Policy applies to all information that we have about you and your account. Users should regularly check this page for any changes to its Privacy Policy.
Your Choices Regarding Your Personal Information

Participants may also contact GoMo Health directly, via email, at support@gold-group.com. Upon your request, we will deactivate your account and contact information from our active databases. Such information will be deactivated as soon as practicable based on your account activity and accordance with our deactivation policy and applicable law. To make this request, either go to your account settings (if applicable) or contact us as provided above. We will retain in our files some personal information to prevent fraud, to troubleshoot problems, to assist with any investigations, to enforce our Terms of Use and to comply with legal requirements as is permitted by law. Therefore, you should not expect that all your personal information will be completely removed from our databases in response to your requests. Additionally, we keep a history of changed information to investigate suspected fraud with your account.
